What is the NHHA System of Integrated Care™

The NHHA System of Integrated Care™ is a continuum of care that provides a holistic approach to addressing the interconnected needs of vulnerable individuals by offering a spectrum of services tailored to their unique circumstances. We recognize that a single solution is rarely enough; instead, we provide a fully integrated network of support to ensure no one falls through the cracks. This approach is achieved not only through a coordinated effort, but through the technology of an integrated Electronic Health Record system and care management platform.

Key Components of Our Integrated Continuum of Care

Referral Pathways
Our team is committed to  meeting individuals where they are—whether on the streets, in shelters, or through referrals within the community. By collaborating with trusted partners and organizations, we ensure that individuals are seamlessly connected to essential services that address their specific needs. Through genuine connections and trust-building, our outreach remains inclusive, culturally responsive, and adaptable, ensuring no one is left behind.

Assessment and Case Management
Upon intake, each client undergoes a comprehensive assessment to identify their specific needs, from physical and mental health to substance use disorder counseling, housing, healthcare, behavioral health, and more. We work hand-in-hand with clients to create personalized care plans and guide them to available resources, ensuring they receive appropriate and effective support. Our team addresses client barriers and celebrates milestones along their journey. With a focus on building trust and resilience, we work to empower individuals to achieve long-term stability.

Stabilization & Healing
Many clients experiencing housing instability are referred to Recuperative Care, a core program that provides a safe and stable environment to heal after hospitalization. Clients benefit from a social community, skill-building workshops, and access to case management, housing services, and employment support. Connections to healthcare services such as primary care, behavioral health support, and substance use treatment are also available to participants of the program.

By partnering with healthcare providers, we ensure that clients experience continuity of care, even as they move into stable housing. This stage of their journey fosters both recovery and long-term stability and serves as a bridge between hospital and home, reducing unnecessary ER visits and healthcare costs by providing medical supervision, case management, and housing assistance while in our care.

The Pathway Home
For those facing homelessness, we provide access to safe and stable temporary housing options. Our transitional programs go a step further, helping clients develop life skills, financial literacy, and job readiness—key components for successfully transitioning into permanent housing.

Embracing the "Housing First" approach, we prioritize placing individuals into stable housing as a foundational step before tackling other challenges. To ensure long-term success, clients remain in Housing Tenancy & Sustaining services for up to a year, receiving ongoing support and resources in order to build a solid foundation. This approach empowers our clients to maintain housing stability and thrive within their communities.
