Ask Me Now is an on-demand coaching service offered in conjunction with in-person training for service providers who need additional advice and support. Ask Me Now subject matter experts provide technical advising relating to the training modules.
Engagement Centers
Engagement Centers promote Basic Population Health Management (BPHM) by connecting Medicaid members to benefits and services they are eligible for at a higher rate.
NHHA’s extensive experience and profound proficiency in the areas of care coordination and facilitating members' access to Social Determinants of Health (SDOH)-related services, furnish us with a framework for lending invaluable support to Managed Care Plans (MCPs). Our approach in partnership with MCPs has been to help close the gap in member encounters directly related to HEDIS measures and, in California, CalAIM member enrollments.
NHHA Community Health Workers (CHW) utilize their comprehensive knowledge to inform members about available services, addressing any concerns or questions they may have. Moreover, they work towards eliminating barriers that may hinder members from accessing these vital services.
The primary objective is to link members with appropriate services aligned with specific HEDIS measures and increase Enhanced Care Management (ECM) and Community Supports (CS) enrollment rates.
